Salveo Direct Care was born from a desire to take back medicine and deliver personalized health care in a revolutionary way. Four years ago, as co-owner and founder of a traditional primary care practice here in San Antonio, Dr. Prisiliano grew weary of the traditional model of primary care- one dictated by insurance regulations, deductibles, and copays.

He felt deeply there was a better way to care for his patients and soon discovered Direct Primary Care- a membership-based medical model that provides concierge-level care at a fraction of the price. Salveo Direct Care opened in August of 2018 and has been growing ever since. When the timing was right for their family, his wife and fellow Family Physician, Dr. Monica joined the practice in April of 2021.

In the early days of the pandemic, we both felt another way to disrupt the medical industry was to build a practice focused on well care rather than sick care. Although we certainly care for acutely ill patients and those with chronic disease, we knew that an ounce of prevention is, as they say, worth a pound of cure.

With this wellness mindset, we both became certified in Lifestyle Medicine as well. We are San Antonio’s premier boutique Lifestyle and Direct Primary Care clinic, and we serve people of all ages and walks of life- individuals and employers alike. Our mission is to make quality, personalized, and easily accessible healthcare available to everyone.

We all face challenges, but looking back would you describe it as a relatively smooth road?
With any new business, particularly in the midst of a pandemic, there are always struggles. Fortunately, due to our practice structure and the nature of Direct Primary Care, we have been able to face those challenges head-on and weather the storm of COVID.

Given the direct agreement we have with our patients, we aren’t subject to insurance restrictions and are able to be more flexible with how we provide services to our patients. We have utilized telemedicine and secure messaging from the beginning, which has allowed us to tailor our patients’ care in a way that is not achievable in conventional practice.

Although running and owning a business while practicing medicine is certainly challenging, the rewards are immense. As Lifestyle and Direct Primary Care physicians, we are able to practice medicine the way it was always intended to be practiced, restoring the doctor-patient relationship.
