Hi Dr Manuel , it’s an honor to have you on the platform. Thanks for taking the time to share your story with us – to start maybe you can share some of your backstory with our readers?
I met my wife who is also a physician in Brooklyn NY during residency at NYU Langone Lutheran Medical Center. She is originally from San Antonio TX and I am from Freiburg Germany. After residency we moved to San Antonio TX. I co-owned a medical group called San Antonio Medical Associates which was a insurance based primary care practice. After 7 years of insurance “volume based” practice I decided to change to Direct Primary Care. I was able to stay in Alamo Heights were I practice ever since we moved to San Antonio in 2016 and opened San Antonio Direct Primary Care Alamo Heights.

I’m sure it wasn’t obstacle-free, but would you say the journey has been fairly smooth so far?
The biggest struggle has actually been the rapid growth. Patients absolutely love the DPC model. I am currently looking for another physician to join my practice and have recently been in talks with a pediatrician who is also direct care based to lease one of my spare rooms

Thanks – so what else should our readers know about SADPC Alamo Heights?
In August 2023 I started San Antonio Direct Primary Care Alamo Heights. It is a purely membership based practice (I do not bill insurance) where I charge $120 per month and this includes all visits with me whether in person, virtual or via text. We include flu/strep/covid testing, EKGs, Large Joint Injections, Trigger finger injections, diabetic nail trimming, ear wax removal, some skin biopsies, breathing treatments, anti inflammatory injections, B12 injections and few other minor procedures. There is no additional cost or copays for these or my visits. We also draw labs in the office and patients can use their insurance or opt for our wholesale cash pricing for these (example basic annual panel is $25 cash). The model let me go from a patient panel of 4000 patients to just a few hundred. I can now spend up to an hour or more with patients if I need to without being rushed. I treat many conditions including high blood pressure, high cholesterol, low thyroid, arthritis and weight management including GLP medication. One of my passions though is treating diabetes. It has been amazing with the new medications such as Mounjaro and Ozempic to watch patients actually no longer require insulin injections.

What do you like best about our city? What do you like least?
I love that it is a large city with a small town feel. Many patients continue to struggle with insurance and surprise bills but that is a national issue. Access to care is also difficult. Many patients sign up for obama care plans and in Texas the only options are really HMO plans with terrible access and many specialists don’t accept them. I once had a patient when I was still in insurance based practice and there was only one neurologist over 1 hour away who took his HMO and his next appointment was in 8 months. Many times I also have patients who use their insurance for labs. I recently had a patient show me a bill after insurance adjustments that was $200 the same labs cash price at my clinic are $25
